About Intravenous Therapy Card Renewal

Please be reminded that there are NO BACK TO BASIC IVT TRAINING if your IVT card is expired.

For IVT card holders with 2014 expiration, you can renew your IVT card through:
1. Attend the 3 IVT Updates/ Seminar
2. Submit requirements for renewal:
a. Old IVT card
b. Original and photocopy of 3 IVT Updates certificate 
C. Payment: Renewal Fee: P400, Courier fee: P50

For IVT card holders with 2013 and below expiration, you can renew your IVT cards through:
1. Attend the 3 IVT Updates/ Seminar
2. Submit requirements for renewal:
a. Old IVT card
b. Original and photocopy of 3 IVT Updates certificate 
c. Payment: Renewal Fee: P400, Courier fee: P50
d. PENALTY FEE: P500
